In order to participate in any practice, game, workout, evaluation at Pisgah you must follow these steps.
Make sure the following FOUR forms have been completely filled out and are on file in the athletic office. A link is provided for each form.
Preparticipation Physical Evaluation: This form must be complete before you will be able to participate in any practice, game or workout. This physical is good for 1 year (12 months) and then expires. A new physical is required every year. The physical must be valid for the entire season. For example, if the physical expires on March 23rd (in the middle of the spring sports season), then the student-athlete must have a new physical before the spring sports season begins.
There are three sections to this form.

Medical History - Page 1 - Must be signed and dated by student and parent

Physical Evaluation- Page 3 - Must be signed and dated by a doctor. You may go to any doctor, but they must use this form.

Athletic Clearance - Page 4 - Must be signed and dated by the doctor who administers the physical.

Waiver/Release : This form must be signed and dated by a parent and the athlete.
Transportation Policy: There are two sections of this document.

Transportation Code of Conduct - Page 2 - Must be signed and dated by student.

Transportation Permissions - Page 3 - Must be signed and dated by a parent.
Activity Fee: Once your child has made a team you will then need to pay the Activity Fee. The coaches will communicate whether your sport is a no-cut or tryout situation. This money is due before the season begins. To pay the Activity Fee or for more information please click here.
